There is one word on a lab report that has always bothered me more than it probably should.
Normal.
It looks so definitive sitting there beside a number. Neat little reference range. Your result comfortably between the brackets. No red flag. No alarming boldface. Nothing apparently requiring anyone to leap from the room shouting for reinforcements.
Good. I am quite fond of uneventful lab work.
But then there is this inconvenient little detail: you still do not feel right.
Maybe your sleep has changed. Your hair is shedding. You are cold when nobody else is cold. Your digestion has slowed down. Your energy has disappeared somewhere around the same time your brain became strangely unreliable. Your periods are doing interpretive dance. Your weight or body composition has changed without a corresponding revolution in your habits.
Whatever the collection happens to be, you have noticed a change.
You explain it. Blood gets drawn. The results come back.
“Everything looks normal.”
And sometimes that is where the conversation ends.
That is where mine began. Because eventually I started wondering about the word itself.
Normal compared with whom?
The Lab Report Is Answering a Narrower Question Than We Think
A reference range is an extraordinarily useful thing. I would very much like laboratories to continue using them.
But we tend to ask that little range to do more work than it was hired to do.
For many laboratory tests, the reference interval represents the values found in a defined reference population, often capturing roughly the middle 95 percent of that group. Other tests use clinical decision limits derived in different ways because a particular threshold has been associated with diagnosis, risk, or treatment decisions.
Already, “normal” is getting less simple.
The green band on your lab report is therefore not a microscopic portrait of how your body functions best. It is a comparison tool. It helps a clinician see whether your result is unusual relative to an appropriate reference group and, depending on the test, whether further investigation may be warranted.
That is a mouthful — and valuable information.
It is simply not all the information.
This became an important distinction for me because I am Joni. I am not a percentile.
Neither are you.
Two people can legitimately have values inside the same reference interval while normally living at different places within it. Many biological measurements also fluctuate within an individual. Age, sex, medications, illness, time of day, menstrual timing, pregnancy, laboratory method and plenty of other context can matter depending on the test.
So, the useful question is not merely, Is the number inside the range?
Sometimes another question deserves to sit beside it:
What has changed from this person’s usual pattern?
That does not make the reference range wrong. It makes the person more interesting than the range.
A Snapshot Can Be Accurate and Still Be Incomplete
This is especially relevant when we are measuring systems that move.
Midlife provides one almost obnoxiously good example.
The menopausal transition is not a synchronized lowering of hormone levels in which estrogen politely descends three points every Tuesday until menopause arrives. Ovarian function becomes more variable. Ovulation becomes less predictable. Estradiol and progesterone patterns change. FSH changes. Menstrual cycles themselves begin carrying useful information about where a woman may be in the transition.
Which means a blood draw can accurately tell you what was happening when the blood was drawn and still fail to describe the whole movie.
That matters because a woman can be having very real changes in cycle pattern, vasomotor symptoms — lovingly known as hot flashes, sleep, mood or other midlife symptoms without one hormone measurement arriving dramatically outside a laboratory range.
It also matters because not every miserable thing that happens after forty is perimenopause.
That would certainly make healthcare more convenient. We could hand out one diagnosis with the birthday cake and be done with it.
Unfortunately, fatigue has many possible explanations. So do sleep disruption, anxiety, changes in weight, brain fog, palpitations and hair loss. Midlife may involve reproductive hormone changes while anemia, thyroid disease, sleep apnea, medication effects, nutrient deficiencies, mood disorders or other medical issues are happening at the same time.
This is why I do not want us choosing between believe the lab and believe the symptoms.
That is a false choice.
I want both pieces of information on the table.
Thyroid Is Where This Became Personal for Me
Thyroid symptoms are where I really learned to pay attention to the difference between population normal and individual change.
And thyroid is also where this conversation can go sideways in a hurry, because the internet has turned a legitimate clinical nuance into an entire cottage industry of declaring that every tired woman with a normal TSH secretly has an undiagnosed thyroid disorder.
I am not interested in trading one oversimplification for another.
TSH is an excellent and sensitive tool for assessing primary thyroid function. If your TSH is normal, that is meaningful information. It makes some thyroid diagnoses less likely, and it absolutely should not be discarded because somebody online announced that doctors “never run the right tests.”
At the same time, thyroid physiology demonstrates something fascinating about reference ranges: individuals tend to operate within narrower thyroid hormone patterns than the broad range seen across a population. A classic longitudinal study followed healthy people over time and found distinct individual thyroid set points, with within-person ranges substantially narrower than the group reference range.
Come look at that for a second.
That does not mean, “My TSH moved and therefore I have hypothyroidism.”
It means longitudinal information can matter.
A value of 3.5 and a value of 1.0 might both appear inside the laboratory interval. Whether that difference has clinical meaning for a particular person depends on the size and persistence of the change, the circumstances of testing, other thyroid measurements when appropriate, medications, symptoms, health history and the clinician’s assessment.
And those famous “thyroid symptoms” do not belong exclusively to the thyroid. Fatigue has apparently refused to sign an exclusivity agreement with endocrinology. Neither have brain fog, constipation, mood changes, weight changes or dry skin.
Symptoms tell us something may deserve attention.
They do not automatically tell us what that something is.
That distinction may be one of the most important pieces of self-advocacy I ever learned.
Your Pattern Is Data Too
For a long time, medicine has had a measurement problem that is not really medicine’s fault.
Blood can be quantified beautifully.
“How I have felt for six months” is harder to put into a tidy box.
So, guess which one tends to look more authoritative?
This is where symptom tracking can change the quality of a medical conversation—not because your notebook outranks the laboratory, but because it turns a vague complaint into a longitudinal pattern.
“I’m exhausted” is important.
“I began waking between 2 and 4 a.m. four or five nights a week in March, my menstrual cycles shortened from 29 days to 22–24 days around the same time, I have had four episodes of night sweating, and my daytime fatigue is consistently worse after those nights” gives a clinician considerably more to work with.
As does:
“My constipation began two months after this medication changed.”
“My resting heart rate has increased compared with last year.”
“My TSH was 1.1, then 1.4, then 3.6 over three years. Is that change meaningful in my situation?”
“My periods have become much heavier, and now I am exhausted. Should we be looking at iron status?”
“I have a family history of autoimmune thyroid disease. Does that change what you would consider?”
Now we are investigating. Patterns do not establish diagnoses. They help us ask better questions.
There is a world of difference.
Being Your Own Advocate Does Not Mean Fighting the White Coats
I am rather attached to the idea of being your own advocate in front of the white coats, but I should probably explain what I mean by that.
I do not mean arriving determined to prove the physician wrong.
I do not mean ordering thirty-seven laboratory markers because an Instagram reel used the words optimal range in a serious tone.
And I certainly do not mean continuing to shop for clinicians until someone finally agrees with the diagnosis we assigned ourselves in the parking lot.
Self-advocacy is participation.
It is knowing your history well enough to contribute it. It is keeping previous results when trends might matter. It is describing symptoms specifically rather than trying to remember six months of biology while sitting on butcher paper under fluorescent lighting. It is asking what a test actually tells us and what it does not.
Sometimes a few questions completely change the conversation:
- What does this result reasonably rule out?
- How does it compare with my previous results?
- Could timing, medication, illness or another factor affect this measurement?
- Given these persistent symptoms, what other explanations should we consider?
- Would repeating this test or adding another test be appropriate in my situation?
- What change would make you want to investigate further?
- If this continues, when should I come back?
That is not challenging the physician’s authority. That is giving the physician better information with which to practice medicine. And a good clinician usually wants that.
I’m Not Replacing “Normal” With “Optimal,” Either
There is another little trap hiding over here.
Once people discover that laboratory reference intervals have limitations, the wellness world often offers a wonderfully marketable replacement:
Optimal.
Well. That sounds better.
Who wants merely normal when we can be optimal?
The problem is that “optimal range” can become just as sloppy if nobody explains where the number came from, what outcome it predicts, whether it applies to people like you, and whether changing it actually improves anything meaningful.
Some biomarkers do have evidence-based targets or clinical decision thresholds associated with better outcomes. That is different.
But attaching the word optimal to a narrower range does not magically transform it into science.
I do not want a prettier yardstick.
I want the right question.
When “Normal” Becomes the End of the Conversation Too Soon
There is a subtle leap that can happen after reassuring test results:
The tests we ran were normal quietly becomes nothing is wrong.
Those are not equivalent statements.
Sometimes the tests really do provide the answer. Sometimes they rule out the concerning possibilities we were looking for, symptoms settle, and everybody goes home happy.
Sometimes the normal result tells us where the answer isn’t, which is also useful.
And sometimes the first round of testing leaves an unresolved pattern that deserves another look.
The person sitting in the exam room should not need a red flag beside a laboratory value before her experience becomes worth describing clearly. But neither should she have to decide, on her own, which disease explains it.
There is a middle ground between passive acceptance and medical vigilantism.
That middle ground is where I think good self-advocacy lives.
Observe carefully.
Keep useful records.
Notice changes from your own baseline.
Understand what the test was actually measuring.
Ask what has been ruled out and what has not.
Stay open to explanations you did not arrive with.
And when your symptoms persist, return with better information instead of assuming that normal means the case has been closed. Because a lab report is one witness. Your history is another.
Your symptoms, timing, trends, physical exam, medications, life stage and other testing may all belong in the same investigation.
Good medicine makes use of all of them.

I simply refuse to ask that little word to tell me more than it knows.
And perhaps that is the more useful question to carry into midlife:
Not only, “Are my labs normal?”
But also,
“Normal compared with whom, what has changed for me, and what question are we still trying to answer?”
That is not distrust of medicine.
That is participation in it.

FAQ
Can you have perimenopause symptoms even if hormone blood tests are normal?
Yes. Hormone concentrations change throughout the menstrual cycle and become increasingly variable during the menopausal transition. One measurement may therefore provide limited information about the larger hormonal pattern. Menstrual history, age, symptoms and clinical context can be important in assessing the transition. Other causes of similar symptoms should still be considered.
Can I have thyroid symptoms with a normal TSH?
You can have symptoms commonly associated with hypothyroidism while your TSH is within its reference interval, but those symptoms are nonspecific and do not establish thyroid disease. TSH remains a useful and sensitive test for primary thyroid dysfunction. Persistent or changing symptoms deserve clinical evaluation so your provider can consider previous results, other causes and whether additional thyroid evaluation is appropriate in your particular situation.
Does “normal” on a lab report mean “optimal”?
Not necessarily—and “optimal” does not have one universal medical meaning either. Some tests have evidence-based clinical targets or decision thresholds. For others, claims about an “optimal range” may be based on limited evidence or simply on someone’s preferred interpretation. Ask what the range represents and whether a different target has been shown to improve meaningful health outcomes.
Should I ask for a full thyroid or hormone panel if I still feel off?
Not automatically. More testing is useful when it answers a sensible clinical question. Your symptoms, age, menstrual pattern, health history, medications, family history, prior laboratory results and physical examination should help determine what—if anything—should be added or repeated.
What should I track before seeing my healthcare provider?
Track what changed, when it began, how often it happens, anything that consistently worsens or improves it, menstrual or bleeding changes when relevant, sleep changes, medication or supplement changes and other objective information you already have. Bring previous laboratory results when possible. The goal is to provide a useful timeline, not to prove a diagnosis.
